Your medical summary, ready for any doctor.
ChartCard is a health summary your family keeps current, backed by a wallet card. Any doctor, nurse, or pharmacist can scan it and get a printable list — even when their office doesn't share your records. No app to install.
- Prescriptions, over-the-counter drugs, and supplements — as they're actually taken
- Allergies, conditions, doctors, and emergency contacts
- Shows who last checked each item, and when
Your doctors don't all see the same chart.
When your doctors work in different medical groups or use different record systems, each one sees only part of the picture. Pharmacy history helps, but it misses a lot.
| What one office may see | ChartCard |
|---|---|
| Prescriptions from their own group | Every medication, from every doctor |
| No over-the-counter drugs or supplements | Over-the-counter drugs, vitamins, and supplements |
| The prescribed dose | The real dose — even "half a tablet" |
| Old prescriptions look current | Stopped medications are marked, and reactions become allergies |
| No date anyone checked it | "Verified by Tim (son), 3 days ago" |

Wherever you get care
Your doctors don't always share records. ChartCard goes with you, so everyone works from the same list.
Specialists in different groups
Your primary care doctor, oncologist, and cardiologist may each see only their own part of the chart.
A new doctor or a second opinion
Skip the clipboard. Hand over a complete, current list.
Urgent care and the ER
When there's no time to call around, the card has what staff need.
The pharmacy counter
Let the pharmacist review everything you take, including vitamins and supplements.
Away from home
Traveling or visiting family? The record works anywhere there's a web browser.
Hospital stays
Give the care team the real list on the way in, and update it on the way out.
How it works
Build the record
Add medications, allergies, conditions, doctors, and emergency contacts. Family members can help keep it current.
Carry the card
We print and mail a laminated wallet card with your photo on it — or your loved one's.
Show it anywhere
At a specialist, a new doctor, urgent care, or the ER, staff scan the card, type the pass phrase and date of birth, and get a list they can read and print. Nothing to install.
What's on the card
Small enough for a wallet. Everything a doctor or nurse needs to find the record.
- Photo, so staff know it's the right person
- Name and date of birth: Jane Sample, Jan 1, 1940
- Record number: CC-0000-0000
- Pass phrase — staff type it, with the date of birth, to open the record: sample only
- QR code to scan
- Emergency contacts: Alex Sample (son)
- Website, for typing it in instead
Built for families sharing the load
Caring for someone is rarely a one-person job. ChartCard keeps everyone on the same list.
Invite family members
Brothers, sisters, and other caregivers can update the record from their own accounts.
See who changed what
Every item shows who last reviewed it and when. Nothing is ever lost.
Gentle reminders
If the list hasn't been checked in 30 days, we'll remind you.
Managing for a parent?
You can set up and run the account on their behalf.
Private by design
Can't be found by guessing
A web address alone shows nothing. Opening the record takes the pass phrase and date of birth from the card.
You'll know every time
We email you the moment the record is opened, with the time and approximate location.
Readers can only read
Nobody who scans the card can change the record or add anything to it.
We never sell your data
No ads, no data brokers. Ever.
For doctors, nurses, and pharmacists
Scan the QR code on the back of your patient's card, or enter the record number below. Then type the pass phrase and the patient's date of birth from the front of the card. The record is read-only and prints on standard letter paper.
How ChartCard works for clinicians →FAQ
Who is ChartCard for?
Anyone whose doctors don't all share the same records — for example, a primary care doctor, an oncologist, and a cardiologist in different medical groups. It also helps at urgent care, in the ER, at the pharmacy, with a new doctor, or when you're away from home.
Does anyone need to install an app?
No. Family members use ChartCard in a web browser, and you can add it to your home screen if you like. Doctors, nurses, and pharmacists only need a web browser on a phone or computer.
My parent can't manage an account. Can I do it for them?
Yes. You can set up and run the account on their behalf. Every change you make is marked with your name, so everyone knows who updated the list.
What if the card is lost?
Anyone holding the card can open the record, so treat it like an ID card. You'll get an email every time it's opened, and you can replace the card at any time, which immediately stops the old one from working.
Is ChartCard a medical record?
No. It's a summary kept by the patient and their family. It hasn't been checked by a doctor, and the card says so. It's meant to help clinicians ask the right questions, quickly.
What happens if we stop paying?
The card keeps working. Anyone with the card can still open the record, but it can't be updated, and it shows the date it was last checked.
Who can see the record?
Only people you invite, plus anyone who has the card. You choose which parts of the record are shown when the card is scanned.
